Physical Wellness Test

How is your Physical Wellness?

          1.  Do you get a sufficient amount of sleep every night?

         2.   You have no need to lose 5, 10, 15 or more pounds?

         3.   You are not taking any prescription medications?

         4.   Your family has no history of heart disease, diabetes, or
               Osteoporosis?

         5.   Do you own a pair of tennis/athletic shoes?

         6.   Do you own any exercise equipment?

         7.   Are you on a regular exercise routine schedule?

         8. You have the motivation to work out alone or enjoy instructional videos
             classes?

         9.  Is your BMI (body mass index) within the range suggested by the 
              National Institute of Health?

       10.  Can you think clearly and are productive in your work?

       11.  Do you watch TV no more than 1 hour a day?

       12.  Do you think it is important to be physically active at least 30 minutes
              day?

 

Take the Circle of Life's 5 Test. 

Start at the center of the circle (print Circle) & working your way to the outer edge, fill in one bar with every "yes" answer and do nothing if answer is "no".

After completing all questions in each area of Life's 5 evaluate how balanced and full you are.  Bring your test in to a Life's 5 coach for us to guide you and create a plan of wellness in one or all 5 areas.
